Introduction: Congenital myasthenic syndromes refer to a heterogenic group of neuromuscular transmission disorders. CMS-subtypes are diverse regarding exercise intolerance and muscular weakness, varying from mild symptoms to life-limiting forms with neonatal onset. Long-term follow-up studies on disease progression and treatment-response in pediatric patients are rare.Patients and Methods: We analyzed retrospective clinical and medication data in a cohort of 32 CMS-patients including the application of a standardized, not yet validated test to examine muscular strength and endurance in 21 patients at the (...) last follow-up. Findings obtained in our cohort were compared with long-term follow-up studies of CMS-cohorts from the literature by considering the underlying molecular mechanisms. Outcomes of CMS-ST were compared to results of normal clinical assessment.Results: Thirty-two pediatric patients with defects in eight different CMS-genes were followed by a median time of 12.8 years. Fifty-nine percentage of patients manifested with first symptoms as neonates, 35% as infants. While 53% of patients presented a reduced walking distance, 34% were wheelchair-bound. Even under adequate therapy with pyridostigmine and 3,4-diaminopyridine, CHAT-mutations led to the progression of muscular weakness partly in combination with persistent respiratory and bulbar symptoms. RAPSN, CHRND, and CHRNB1 patients with neonatal manifestation, early respiratory problems, and bulbar symptoms showed a good and maintained treatment response. CHAT and CHRNE patients required higher PS dosages, whereas RAPSN patients needed a lower mean dosage at the last follow-up. The benefits of short-term medication and long-term progression of symptoms were highly dependent on the specific genetic defect. CMS-ST was carried out in 17/21 patients, determined affected muscle groups including bulbar and ocular symptoms, some of which were not reported by the patients.Conclusions: Our findings and comparison with the literature- suggest a better treatment-response and less severe progression of symptoms present in patients suffering from mutations in CMS-genes directly associated with receptor deficiency, while patients with defects leading to synaptopathy and presynaptic defects tend to have worse outcomes. Assessment of affected muscular groups and clinical symptoms by CMS-ST may be a useful tool for optimal therapeutic management of the patients, especially for future clinical studies. (shrink)

This article presents an interdisciplinary analysis of the glamorization of the courtesan image as proposed by Baz Luhrmann’s film Moulin Rouge. The film sparked the appearance of high-street fashion inspired by the image of the 19th-century Parisian courtesan, which prompted the authors to examine how and why such images might appeal to female consumers. The critical analysis reaches beyond the images themselves to identify and discuss the modes of circulation of such images, and their function in achieving both the material (...) ends of capitalism and the promotion of one of the system’s core values. Moreover, the article hopes to illustrate the possibilities offered by integrating cultural and structural analyses of current social phenomena. (shrink)

In his book, History as a Science and the System of the Sciences, Thomas Seebohm articulates the view that history can serve to mediate between the sciences of explanation and the sciences of interpretation, that is, between the natural sciences and the human sciences. Among other things, Seebohm analyzes history from a phenomenological perspective to reveal the material foundations of the historical human sciences in the lifeworld. As a preliminary to his analyses, Seebohm examines the formal and material presuppositions of (...) phenomenological epistemology, as well as the emergence of the human sciences and the traditional distinctions and divisions that are made between the natural and the human sciences. -/- As part of this examination, Seebohm devotes a section to discussing Husserl’s formal mereology because he understands that a reflective analysis of the foundations of the historical sciences requires a reflective analysis of the objects of the historical sciences, that is, of concrete organic wholes (i.e., social groups) and of their parts. Seebohm concludes that Husserl’s mereological ontology needs to be altered with regard to the historical sciences because the relations between organic wholes and their parts are not summative relations. Seebohm’s conclusion is relevant for the issue of the reducibility of organic wholes such as social groups to their parts and for the issue of the reducibility of the historical sciences to the lower-order sciences, that is, to the sciences concerned with lower-order ontologies. -/- In this paper, I propose to extend Seebohm’s conclusion to the ontology of chemical wholes as object of quantum chemistry and to argue that Husserl’s formal mereology is descriptively inadequate for this regional ontology as well. This may seem surprising at first, since the objects studied by quantum chemists are not organic wholes. However, my discussion of atoms and molecules as they are understood in quantum chemistry will show that Husserl’s classical summative and extensional mereology does not accurately capture the relations between chemical wholes and their parts. This conclusion is relevant for the question of the reducibility of chemical wholes to their parts and of the reducibility of chemistry to physics, issues that have been of central importance within the philosophy of chemistry for the past several decades. On the one hand, Austin appreciates truth as the aim not only of science, but also of philosophy. On the other hand, truth in his works is demythized, contextualized, or even relativized. I analyze this ambivalence by considering some aspects of Austin’s thought which may appear close to pragmatism: his claims that the bearer of truth is assertion as a speech act, that speech acts are to be assessed as felicitous and infelicitous before being assessed as true or false, and (...) that the truth/falsity judgment is concerned with assertions in their contexts, including the participants’ goals and knowledge. These claims are, however, argued for in full coherence with a corrispondentist conception of truth. At a closer examination, Austin’s conception of truth appears to be an independent, “heretic” development of that of Frege. In taking distance from Frege, albeit on a Fregean basis, Austin went some length in the direction in which pragmatists too had gone, without actually sharing any properly pragmatist assumption. Negli ultimi anni le ricerche sulla civiltà rupestre del Mezzogiorno medioevale, in seguito agli studi innovativi di Cosimo Damiano Fonseca, hanno condotto alla rivisitazione, attraverso approcci nuovi e con l'ausilio di strumenti sofisticati, di questo fenomeno nelle sue varie sfaccettature. Se, a partire soprattutto dalla fine dell'800, gli studiosi avevano rivolto i loro interessi in specie alle cappelle rupestri e alla loro decorazione pittorica, ovvero all'aspetto, in apparenza il più eclatante, dell'abitato rupestre, di recente l'attenzione si è focalizzata intorno allo (...) studio del paesaggio rupestre stesso e ai suoi valori, inteso come modello insediativo non certamente alternativo, né inferiore, a quello urbano, bensì come elemento specifico all'interno della complessa realtà dell'insediamento umano delle regioni meridionali e della Sicilia tra Tarda Antichità e Medioevo. Come pensavano il tempo i Greci di eta arcaica, prima che Platone ed Aristotele ne formulassero una definizione? Nei miti, il tempo della natura, ciclico e rassicurante, non era messo a confronto con quello umano, imprevedibile anche se inesorabilmente determinato. Il ritmo scandito dal sole non era sovrapponibile a quelle della vita umana, l'avvicendarsi delle stagioni non misurava la durata della stirpe. Solo in eta sapienziale (VII-V secolo a.C.) si comincio a pensare il tempo in modo onnicomprensivo (...) e si avverti l'esigenza di misurarne il flusso. In quell'epoca, Eraclito tratteggio una concezione del tempo assolutamente innovativa, capace di ribaltare le visione del mondo trasmessa dai miti. Penso al tempo delle stagioni in analogia con quello interiore e vide nell'inesauribile creativita dell'anima umana lo specchio dell'infinita potenzialita della natura. La concezione offerta da Eraclito, avversata dai filosofi di eta classica e misconosciuta dalla scienza, trasmette ancora oggi, a chi legge con attenzione i frammenti rimasti della sua opera, un fiducioso messaggio di speranza nella capacita umana di vivere creativamente il tempo. Sulla scia del famoso detto di Eraclito, quella del fiume è sempre stata l’immagine principale per descrivere la natura inafferrabile del tempo; ma il mare, il luogo dove finiscono tutti i fiumi, fornisce una perfetta analogia naturale per (...) la conservazione del «passato in sé» attorno cui Henri Bergson ha costruito la sua teoria della durata e della memoria: niente scompare davvero, e così il passato viene dotato della consistenza e della vivida eternità che la filosofia tradizionale attribuiva all’essere. Intercessori dal panorama letterario, uniti alla considerazione filosofica di alcune immagini ricorrenti – specialmente quelle del relitto e della città sommersa – aiuteranno ad approssimare la rêverie marina al pensiero di Bergson, approssimazione che si compirà nella conclusione. En este trabajo se realiza una propuesta de comunicación transdisciplinar entre las ciencias cognitivas y la educación, tal y como se perfilan en el siglo XXI. En un primer lugar se contextualizan estos procesos de comunicación en el continuum histórico que transita desde los inicios del siglo pasado, con la configuración de la psicología educativa dentro del constructo de las ciencias educativas; pasando por la constitución de las ciencias cognitivas, a mediados del siglo XX que propició la emergencia de la (...) síntesis disciplinar entre las ciencias del cerebro y de la mente, para posibilitar un encuadre arraigado en el presente, y en las propuestas de consiliencia entre neurociencia cognitiva y educación. Desde este presente histórico, se efectúa una propuesta de diálogo entre las distintas epistemes (educativas y cognitivas), en relación con el fenómeno de la educación bilingüe, y en concreto en el contexto intercultural ecuatoriano. La integración de los distintos niveles de análisis que configuran un fenómeno complejo, como lo es todo fenómeno educativo, refleja cómo los intentos transdisciplinares orientados a reconocer la complejidad ontológica de un mismo objeto de estudio, requiere del diálogo entre las distintas epistemologías y metodologías implicadas. La apertura del campo educativo a las dimensiones mentales y cerebrales en su expresión cognitiva, requiere en este s. Introducción: El dormir mal podría ser la causa de hipertensión arterial y de otras enfermedades crónicas no transmisibles.Objetivo: Conocer la calidad de sueño de las personas con hipertensión arterial.Metodología: Se realizo un estudio descriptivo, se aplico el cuestionario de Índice de Calidad de Sueño de Pittsburgh.Resultados: Se evaluó la calidad de sueño en 85 personas con hipertensión arterial, de abril a diciembre del 2021. El 84% tiene un índice de mala y muy mala calidad de sueño.Conclusión: Las personas con hipertensión (...) con mala y muy mala calidad de sueño puede afectar sus cifras de tensión arterial. (shrink)

From 1989 through September 2017, Chile’s highly restrictive abortion laws exposed women to victimisation and needlessly threatened their health, freedom and even lives. However, after decades of unsuccessful attempts to decriminalise abortion, legislation regulating pregnancy termination on three grounds was recently enacted. In the aftermath, an aggressive conservative drive designed to turn conscientious objection into a pivotal new obstacle, mounted during the congressional debate, has led to extensive, complex arguments about the validity and legitimacy of conscientious objection. This article offers (...) a critical review of the emergence of conscientious objection and its likely policy and ethical implications. It posits the need to regulate conscientious objection through checks and balances designed to keep it from being turned into an ideological barrier meant to hinder women’s access to critical healthcare. Die Hauptthese dieses Buches ist es, Krankheit und Gesundheit mit Nietzsche auf zwei verschiedenen Ebenen zu denken, nämlich als Ereignisse des Willens zur Macht in einem zirkulär-relationalen Modell, in dem sie sich gegenseitig behaupten, sowie die Metaebene der Konzeptualisierung anzuerkennen. Es behandelt die Schwierigkeit des begrifflichen Denkens, diese Ereignisse einzuholen, und diskutiert kritisch Modellbildungen, die darauf hinauslaufen, dass Krankheit zu überwinden und als lineares singuläres Ereignis zu verstehen sei. In Nietzsches Spätwerk wird die Figur des "dionysischen Künstlers" als Idealisierung der (...) Gesundheit gemäß seiner interpretierenden Sichtweise der Welt dargestellt. Die Relevanz des Buches liegt darin, eine Analyse der Begriffe von Gesundheit und Krankheit (tragischer und schlechter Pessimismus) in direkter Beziehung zu leiblichen Vorgängen vorzulegen, die sich von metaphysisch-dialektischen Voraussetzungen befreit. Zu diesem Zweck widmet die Untersuchung sich einer intertextuellen Lektüre der fünf Vorreden, die Nietzsche 1886/87 zu seinen früheren Schriften verfasste, und verknüpft sie mit der Interpretation des Nachlasses und weiterer Veröffentlichungen aus seiner letzten Schaffensphase. Workplaces around the world have experienced extraordinary changes to the composition of their workforces and the nature of work. Few studies have explored workers from multiple countries of birth, with multiple religious orientations, working together within a single country of residence. Building on and extending the Work Values Ethic (WVE) literature, we examine 1,382 responses from employees working in three manufacturing companies. Differences were found in the mean WVE scores of groups of respondents from 42 countries of birth. Their WVE (...) scores were strongly associated with their birth countries’ per capita Gross National Product (GNP), and the means of these scores did not change with variations in the respondents’ length of residence in a different country. These results have implications for developing cross-cultural management practices and for improving relationships with employees, with opportunities for increased commitment and, potentially, productivity. This paper develops a largely neglected parallel between prudence and population ethics. Prudence is generally understood to be concerned with the balancing of well-being over time. How, precisely, well-being ought to be balanced over time, however, is a fervently debated question. I argue that developing a standard guiding such evaluations is exceedingly challenging. This is due to an often overlooked fact about prudence, namely that it shares a structural similarity with population ethics: In both contexts, we assess the comparative value (...) of populations of personstages/people, which may vary in number and level of well-being. Based on this analogy, I show that the development of an adequate theory of prudence runs into very similar impossibility results as obtain in population ethics. In particular, I prove that Arrhenius’ fifth impossibility theorem can be applied to prudence. I develop and compare four possible answers to this challenge. First, I discuss the possibility of accepting the very repugnant conclusion intrapersonally. Second, I present and further develop Donald Bruckner’s Minimax Regret approach, which gives up Transitivity. Third, I apply Jacob Nebel’s Lexical Threshold View to prudence and critically evaluate it. Lastly, I introduce what I call the Negative Lexicality View, which is based on Lexical Threshold View but overcomes some of its problems. (shrink)

This article brings together the history of the social sciences and the history of social thought in Socialist Romania. It is concerned with the development of ideas about the social beyond collectivism, especially about the relationship between individual and society under socialism, from the early 1960s to the end of the 1970s. The analysis speaks to three major themes in the current historiography of Cold War social science. First, the article investigates the role of disciplinary specialization in the advancement of (...) new ideas about the social in the postwar period. Specifically, it asks how the debate over the relationship between sociology and Marxism-Leninism has challenged ideas about collectivism from Stalinist social science. Second, the article shows how social practice, individual and collective agency, and people's subjectivities became theoretically relevant in the 1960s, and how they were integrated, via empirical sociological research, into the reworked conceptual apparatus of post-Stalinist Marxism-Leninism. This complicates accounts about the role of quantification and theorization in postwar social science by foregrounding the intense reflection on the role of empirical research in sociology under state socialism. Third, the article shows how the relationship between individual and society became a topic of interest across social sciences in the 1960s and 1970s. The Marxist humanist approach to the social, although it never achieved the institutional status of a distinct discipline, adds an important perspective from East Central Europe to the existing historiography of the ‘thinning’ of the social in social sciences and social thought beginning in the 1950s. It seems natural to think that emotional experiences associated with a memory of a past event are new and present emotional states triggered by the remembered event. This common conception has nonetheless been challenged at the beginning of the 20th century by intellectuals who considered that emotions can be encoded and retrieved, and that emotional aspects linked to memories of the personal past need not necessary to be new emotional responses caused by the act of recollection. They called this specific (...) kind of memories “affective memories” and defended their existence. My aim here is to expound the historical background of this debate as well as the characterization and development of the notion of affective memory since its first inception. I aim to show that although the debate was left unresolved and the term disappeared from academy around 1930, many of the characterizations of the nature of emotions and memory done by the advocates of affective memory have reappeared in the scientific agenda and been further developed during the last decades. The Times Literary Supplement of November 8, 1917, contained, under the title of Socrates recognitns, a review of Plato's Biography of Socrates, a lecture delivered by Professor A. E. Taylor to the British Academy in the early part of last year. The opening sentence of the review is as follows: ‘Next to the problem of the Gospels ranks that of the Platonic dialogues amongst those most vital to the history of the human spirit.’ A little further down the reviewer says: (...) ‘It is much to the credit of British scholarship—and especially to that of the University of St. Andrews—that it should have attacked these problems with untiring energy, and propounded solutions which, although they run counter to most of the traditional tendencies of historical and philosophical criticism, have not only challenged attention, but are carrying conviction even to unlikely quarters.’ And again, at the end of the article, we read this passage: ‘It is scarcely to be thought that the ground won by the scholars of St. Andrews will be held without counter-attack; but this is slow to mature, and in the meanwhile such essays as the subject of this notice, with which we may couple the paper recently read to the British Academy by Professor Burnet on the Socratic doctrine of the soul, serve to buttress and consolidate the position.’. One of the essential tasks of bioethics in morally pluralistic societies consists of fostering a public use of reason in its field of competence. This is the fundamental thesis of the present work. To deal with it I will try to answer two crucial questions: What is moral pluralism and why does it demand fostering the use of public reason in moral issues in the sphere of bioethics and What are the laboratories of bioethics in a pluralistic society?
